Pharmacist I – Massachusetts, Boston
Job Type
- Long-Term Assignment (LTA)
- Contract length: 13 weeks
Job Summary
- Optimize drug therapy by identifying, resolving, and preventing drug-related problems to improve therapeutic outcomes.
- Provide pharmacy services according to state and federal laws and regulations.
- May prescribe medications under specific collaborative drug therapy management agreements (CDTM).
- Does not require direct patient care.
Essential Functions
- Direct and coordinate pharmacy service activities.
- Supervise and verify accuracy of work by support personnel during assigned shifts.
- Operate within pharmacy policies and procedures.
- Ensure compliance with drug programs, formulary management, medication management initiatives, and adverse drug event monitoring.
- Document and report clinical interventions, medication errors, and adverse drug events.
- Participate in department quality improvement efforts.
- Train medical, nursing, and allied health professionals.
- Work relatively independently.
Education & Licensure
- Doctor of Pharmacy (PharmD) required; experience cannot substitute for degree.
- Valid Pharmacist license for Massachusetts.
- Massachusetts Controlled Substances Registration (MCSR) required.
Requirements/Qualifications
- Valid Pharmacist license in Massachusetts.
- Doctor of Pharmacy (PharmD) degree.
- Minimum of 1 year of pharmacist experience.
- Pharmacist experience within the last year.
Required Credentials & Compliance
- Documentation of immunity or vaccination for measles, mumps, and rubella (MMR) or proof of immunity via blood test.
- TB screening documentation within 3 months of start date; updated if break >6 months.
- Physician health statement within 1 year of start date; updated if break >6 months.
- Criminal background check within 45 days prior to start; updated if break >6 months.
- Massachusetts CORI background check within 30 days prior to start; updated if break >6 months.
- Resume including past 7 years of work experience with city and state.
- Two professional references within 4 years, including at least one supervisor or higher.
- Skills checklist completed within 1 year of submission.
- Confirmation of assignment/POI form required for initial assignments and extensions.
- Education verification through background check.
- Social Security Number trace as part of background check.
- National sex offender registry check within 30 days of start; updated annually.
- Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days of start; updated annually.
- Candidate submission coversheet required for initial assignment submissions.
- Attestation completed upon registration in the applicant system.
Optional but Recommended
- Documentation of chest x-ray if positive TB test.
- Name change documentation if legal name differs from credential documents.
- TB symptom survey if positive PPD, updated annually.
This position requires a licensed pharmacist with a PharmD degree, preferably with 2-3 years of experience, who can work independently and ensure compliance with pharmacy policies and regulatory requirements. The role focuses on pharmacy operations and quality improvement rather than direct patient care.
