Operating Room Registered Nurse Travel/Contract Job - Indiana
Location
- City: Avon, Indiana
Job Type
- Travel/Contract assignment
- Contract length: 13 weeks
Shift Details
- Day shift, 3 shifts per week
- 12-hour shifts (7:00 AM to 7:30 PM)
- Call required: No more than 40 hours of call in a 6-week period with a 45-minute response time
Unit & Department Information
- Operating Room with 8 total OR suites
- Specialty areas include General Surgery, Gynecology (GYN), Urology, Orthopedic Total Joint/Sports Medicine, ENT/PLS, Endovascular, Podiatry, and DaVinci robotic surgery
Requirements/Qualifications
- Valid RN license in Indiana
- Minimum 5 years of Operating Room (OR) experience, including OR experience within the last year
- Completion of at least one travel nursing assignment
- ACLS certification issued by the American Heart Association
- BLS certification issued by the American Heart Association
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Experience in Orthopedics
- Experience in Urology
Licensure & Certifications
- Indiana or Compact RN license required and must be in hand at time of submission
- Certifications required before start:
- ACLS (AHA or RQI accepted)
- BLS (AHA or RQI accepted)
Prior Travel Experience
- First-time travelers NOT accepted (prior travel experience required)

- No floating required
Facility & Work Environment
- Short-Term Acute Care facility
- Charting system: Cerner
- Scrub color: Red top, black bottoms
- Free parking available
Interview Process
- Interviews conducted in the afternoons
- Candidates must provide 2–3 available time slots for interview scheduling
Required Documentation & Compliance
- Valid Indiana Driver’s License or Passport (clear copy)
- Background check within 90 days including 7 years of residence/work history, SSN trace, county/state/national checks, education verification, and international search if applicable
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
- Official transcript or diploma verifying degree awarded and graduation date (may be pending at start)
- Education verification by third party (may be pending at start)
- Physical exam within 12 months of start date
- Fit mask test within 12 months with approved respirators (3M N95 models or Honeywell N95)
- Negative 10-panel drug screen within 12 months (amphetamines, marijuana, cocaine, opiates, PCP; instant tests not accepted)
- Immunizations and titers required:
- Rubeola (Measles): positive titer or 2 vaccines (no declinations)
- Mumps: positive titer or 2 vaccines (no declinations)
- Rubella: positive titer or 2 vaccines (no declinations)
- Varicella: positive titer or 2 vaccines (no declinations; history not accepted)
- Hepatitis B: series, titer, or signed declination required
- Tetanus/Diphtheria/Tdap: strongly recommended (preferred)
- Flu vaccination/documentation required during flu season (9/30 - 3/31); no declinations accepted
- TB testing within 1 year (PPD, QuantiFERON-TB Gold, or T-SPOT) with chest x-ray if positive history
- Annual TB questionnaire required
- Color blind exam (Ishihara or equivalent) with minimum 100% score on online test
- Medication/pharmacology competency exam and unit competency exam required
- References: minimum 2 from recent employment within 2 years; must be from charge nurse, manager, supervisor, or above
- Current employment application with explanation of any gaps over 30 days within last 7 years
Orientation & Facility Specific Requirements
- Completion of facility-specific forms and packets prior to start, including:
- Self Study Orientation & Test (must be completed via DocuSign at least 1 week prior to start)
- Compliance checklist and confidentiality agreements
- Look of Assurance packet
- Policy information and job description documents
- HIPAA acknowledgment updated annually
