Description
Home Care Registered Nurse
Location: Rutland, Vermont
Employment Type: Contract (13 weeks)
Shift: 8am to 4pm Monday - Friday, with some on-call and weekend requirements
Hours per Week: 37
Job Overview
- Provide quality nursing care to assigned patients using the nursing process and evidence-based practice.
- Serve as Case Manager for assigned patients, managing caseloads and visit times according to productivity standards.
- Develop, communicate, and implement realistic, individualized plans of care involving patients and caregivers.
- Perform comprehensive patient and caregiver assessments and participate actively in interdisciplinary team (IDT) meetings.
- Coordinate patient care with physicians, providers, team members, and vendors to ensure safe and effective care.
- Manage patient discharges and transfers, ensuring all paperwork and information comply with policies and regulations.
- Maintain accurate, timely documentation including OASIS and other required state and federal forms.
- Educate patients and caregivers on medication side effects, treatment options, discharge instructions, and reportable conditions.
- Advocate proactively for patient care needs to ensure sensitivity to individual patient and family circumstances.
- Participate in monthly peer review audits and case conferences to support quality improvement and team communication.
- Utilize and maintain high-tech nursing skills such as IV use and maintenance, wound VAC, and complex wound care.
- Ensure infection control standards are maintained throughout the plan of care.
- Collaborate with Licensed Practical Nurses (LPNs) within their scope of practice to delegate tasks and ensure care plan adherence.
Required Qualifications
- Valid Registered Nurse (RN) license in Vermont.
- Minimum of 2 years of Home Health nursing experience, including experience within the last year.
- Basic Life Support (BLS) certification.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Experience with OASIS documentation.
