Description
American Traveler is seeking an experienced Hospice RN for a community and inpatient hospice setting in Vermont, requiring a minimum of 3 years of hospice experience and Epic proficiency.
Details
- Home health and inpatient hospice setting serving both community-based patients and a 21-bed inpatient respite house (MMRH)
- Total hospice census of approximately 100 patients: 21 inpatient at MMRH and approximately 80 in home, SNF, or ALF settings
- Patient population is primarily terminally ill adults (6-month prognosis or less), with occasional pediatric patients in the home setting
- Community/home hospice RNs carry caseloads of 12–14 patients; MMRH day nurses care for 5–7 patients, overnight shifts up to 11
- 10-hour day shifts (7:00 AM – 5:30 PM), 40 hours per week
- Schedule rotates between home hospice and MMRH, including a weekend rotation; Monday through Sunday availability required
- MMRH shift hours are 7:00–7:30 (12 hrs); home hospice is 8:00 AM–4:30 PM M–F with weekend rotation
- On-call coverage is rarely needed but candidates may be asked to cover the on-call team
- Holiday coverage required if needed
- No floating required; however, cross-setting training (home hospice and MMRH) is preferred
- No block scheduling
- CNA/PCT support available (4 per shift); a full-time NP is assigned to the program, primarily at MMRH
- EMR: Epic (required); Net Smart used for documentation (training provided during orientation)
- Special equipment includes CADD pumps, IV pumps, BiPAP, CPAP, cough assist machines, nebulizers, Coagucheck for INR monitoring, GoMco suction pumps, and feeding pumps
Requirements
- Active VT or Compact RN license required
- NY license with proof of pending application will also be reviewed
- Current BLS certification (AHA) required
- Minimum 3 years of community hospice experience required
- Epic EMR experience within the past 2 years is required and non-negotiable
- Strong typing skills required due to a narrative-heavy EMR
- Proficiency in complex symptom assessment and management with high-level critical thinking
- Experience with medication management and administration via IV, subcutaneous, oral, sublingual, rectal, and other routes
- Comfortable initiating and participating in goals-of-care discussions with patients, families, and care teams
- Experience with central line care including PORT/IVAD, PICC, subcutaneous site insertion, palliative sedation, tracheostomy management, wound care, and venipuncture
- Two professional references required: at least one from a nurse manager or supervisor within the past 6 months, and one from within the past year
- SSN and DOB required for consideration
- All employment gaps over 90 days must be explained
- Complete work history including bed count, EMR, and facility type required for consideration
- Reliable personal transportation required — public transportation is not accessible in this area; all-season or snow tires recommended November through April
Additional Information
- Provides end-of-life care including comprehensive assessment, care planning, and symptom management for terminally ill patients and their families
- Traveler case managers are typically assigned a specific geographic area; weekend rotation coverage may extend across Chittenden and Grand Isle Counties
- Performs unit-specific procedures including continuous pain infusion initiation, palliative sedation, rectal tube management, ostomy and drain management, and wound care
- Educates patients and caregivers on aspects of end-of-life care with strong communication skills
- 2–3 days of orientation provided; facility onboarding is completed online prior to arrival
- Dress code: business casual or non-institutional colored scrubs; closed-toe/supportive shoes; limited jewelry; neat hair and beard; no nail polish
- First-time travelers are welcome to apply
- This is a teaching facility, though teaching hospital experience is not required
