Description
Physical Therapist - Outpatient
Location: Nantucket, Massachusetts
Employment Type: Long-Term Assignment (approximately 17 weeks)
Setting: Outpatient only
Shift: 8:00 AM – 4:30 PM (4 shifts per week, 10 hours each)
Job Details
- Outpatient physical therapy position with shifts starting between 8:00 and 8:30 AM
- Shared housing provided at no cost
Required Qualifications
- Valid Physical Therapist license in Massachusetts
- Minimum of 1 year of Physical Therapist experience, including experience within the last year
- Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
- FEMA ICS 100 Training (must be completed before onboarding)
Experience
- No explicit years of experience stated beyond licensure requirement
- No prior travel experience required
Required Onboarding and Compliance Items
- Professional license verification (primary source verification required; conditional licenses not accepted)
- BLS certification
- FEMA ICS 100 Training (must be completed before onboarding)
- Fit Test for N95 respirator within 365 days of start date (no exemptions allowed)
- Respirator Medical Clearance Attestation (if fit test required)
- MMR vaccination or proof of immunity
- TB test documentation within 3 months of start date (skin test or blood test)
- Health Statement signed by MD/DO/NP/PA within 365 days of start date
- Criminal background check within 45 days prior to start date, including Massachusetts CORI and national sex offender registry checks
- Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists within 30 days of start and annually
- Resume including past 7 years of work experience with city and state
- Two professional references within 4 years (minimum one supervisor-level)
- Skills checklist completed within 1 year of submission
- Confirmation of Assignment/POI form upon acceptance and for any extensions
- Education verification through background check company
- Social Security Number trace as part of criminal background check
- Candidate submission coversheet for initial assignment submission
Additional Notes
- Housing agreement form required if staying in provided housing, especially if there is a break of 35 days or more between assignments
- Fit testing cannot be done onsite; must be completed prior to start or at designated locations if extending assignment
- Positive TB test requires documentation of a negative chest x-ray (optional but recommended)
- Name change documentation required if legal name differs from credential documentation
