Job Opportunity: Registered Nurse - Home Health

Job Description

Provide and coordinate home nursing care according to physician orders and agency policies.
Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Average patient load: 5-8 patients per day.
Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
Must be willing to travel up to 75 miles from the home clinic, averaging 150-200 miles per week.
Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours).
Dress code: Scrubs.
Charting system: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (for RNs).
Weekend and on-call requirements vary; discuss during interview.

Required Qualifications

Valid Registered Nurse (RN) license in Massachusetts.
Minimum of 1 year of recent experience in Home Health nursing, including experience within the last year.
Experience working with adult patients in a Home Health setting.
Hospice nursing experience.
BLS certification issued by the American Heart Association or American Red Cross.
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Proficiency with HomeCare HomeBase (HCHB) documentation system.
Valid driver's license and reliable transportation.
