Physical Therapist - Home Health (Travel/Contract)
Location: Chesapeake, Virginia
Contract Length: 13 weeks
Job Details
- Specialty: Therapy/Rehabilitation (Physical Therapy)
- Employment Type: Travel/Contract
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours)
- Average Patients per Day: 5-6
- Weekly Productivity Expectation: Approximately 30 units (points) per week
- Typical Caseload: 5-10 patients per day, including Start of Care and Follow Ups
- Travel: Must be willing to travel up to 75 miles from home clinic covering Suffolk, Franklin, Windsor areas; average 200 miles per week
- Work Setting: Home Health with home visits and periodic office meetings
- EMR System: HCHB
- Weekend/On-Call: No weekend rotation or on-call required
Required Qualifications
- Valid Physical Therapist license in Virginia
- Minimum of 1 year of Physical Therapist experience within the last year
- Experience in a Home Health setting
- Completion of at least one travel assignment
- Current BLS certification issued by the American Heart Association or American Red Cross
- Valid Driver’s License
Compliance and Documentation Requirements
- Background checks covering last 7 years of residence and employment, including state and federal databases, sex offender registries, and motor vehicle records
- State-specific clearances and fingerprinting as required by Virginia and other states in the coverage area
- Education verification for applicable degree
- Completion and signing of confidentiality, privacy, compliance, and code of conduct documents prior to start
- Completion of home health-specific policy acknowledgements and quizzes prior to start
- Proof of personal or auto insurance valid and current, with candidate listed as insured or driver
- References: Two professional references required
- Work history verification for at least the last 7 years or two most recent employers
Additional Notes
- No guaranteed hours; if patient census is low, contractor may be floated to a nearby clinic to pick up hours
- Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not reimbursed)
- Local candidates accepted at the same rate
- Weekend and on-call requirements may vary by location; discuss during interview if applicable
This position is ideal for experienced Physical Therapists with recent home health experience who are comfortable with travel and working independently in a home health setting.
