Job Summary:
The Physical Therapist (Home Health) position located in Saluda, Virginia, involves delivering therapeutic services to patients in a home health setting. The role requires assessing patient needs, developing treatment plans, and providing therapy in accordance with physicians orders. Job Details:

• Certifications/Education/Experience: Successful completion of a physical therapy program approved by the American Physical Therapy Association, with 2-3 years of physical therapy experience preferred. Current and unencumbered license to practice physical therapy specific to Virginia is required.
• Schedule: Monday through Friday, 8:00 AM to 5:00 PM, minimum of 40 hours per week.
• Required Qualifications: Current CPR certification; familiarity with general computer use including login procedures, internet, email, and navigation of computer desktops; ability to self-manage online HR services and training programs.
• Additional Information: The role involves home visits and periodic office meetings. Candidates must have reliable transportation, a current drivers license, and agency-required liability insurance.
• Location: 1752-Brookside Home Health, 18 New Street, Saluda, VA 23149.
• Travel: Approximately 200 miles per week.
• EMR System: HCHB (Homecare Homebase).

• Shift Hours: Monday to Friday, 8:00 AM to 5:00 PM.
