Description
Job Opportunity: Registered Nurse - Home Health (Travel/Contract)
Location: Phoenix, Arizona
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
Hours per Week: 40
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
- Average patient load: 5-8 patients per day.
- Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
- Must be willing to travel up to 75 miles from the home clinic to patient homes.
- Mileage reimbursed at IRS standard rate (patient-to-patient travel reimbursed; commute to/from first and last patient is not billable).
- Charting system: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (for RNs).
- Dress code: Scrubs.
- No guaranteed hours; if patient census is low, floating to nearby clinics may be arranged.
- Weekend and on-call requirements vary by location and should be discussed during the interview.
- Local candidates accepted at the same rate.
Required Qualifications
- Valid Registered Nurse (RN) license in Arizona.
- Minimum of 1 year of recent Home Health nursing experience within the last year.
- Experience in a Home Health setting; Hospice experience is also required.
- Completion of at least one travel nursing assignment.
- BLS certification issued by the American Heart Association or American Red Cross.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Proficiency with HomeCare HomeBase (HCHB) and OASIS documentation systems.
- Current driver's license and reliable transportation.
Travel and Location Details
- Territory primarily covers Benton and Franklin counties.
- Estimated travel: 100-150 miles per week.
- Home visits with periodic office meetings.
Compliance and Documentation Requirements
- Background checks covering the last 7 years, including SS trace, county, nationwide, sex offender, and state-specific searches.
- Drug screening: 10-panel drug test required.
- Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Immunization or titer proof required for: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, and seasonal flu vaccination or declination.
- COVID-19 vaccination series or declination required.
- Education verification for nursing degree required.
- Two professional references required.
- Verification of work history for at least the last 7 years or two most recent employers.
- Various state-specific releases, attestations, and compliance forms must be reviewed and signed prior to start.
- Confidentiality, privacy, HIPAA, and compliance training must be completed before assignment start.
- Personal or auto insurance policy required, must be current and under candidate’s name or list candidate as a driver.
Additional Notes
- No weekend rotation or on-call required for this assignment.
- Schedule may vary in some territories (e.g., some western territory positions may have 4x10 shifts Friday-Monday, but this position is standard 5x8).
- Candidates must be prepared to sign multiple compliance and policy documents annually or after any break in service.
This position is ideal for experienced Home Health RNs who have recent specialty experience, are comfortable with travel within a defined territory, and are proficient in home health documentation systems.
