Registered Nurse — Home Health

Job Overview

Temp assignment
Home health role
Full-time schedule: 40 hours per week
Shift: Monday through Friday until shift is complete
Float required
On-call may be required if requested

Required Qualifications

Valid RN license in Tennessee
BLS
Associate of Science in Nursing / Associate Degree in Nursing
Driver's License
1-2 years of experience as a Registered Nurse in a clinical care setting
Home Health (2 years of experience)
Home Health experience within the last year
Experience in a Home Health setting
Experience with Homecare Homebase
Experience with OASIS

Preferred Qualifications

CHPN certification
Bachelor of Science in Nursing

Key Responsibilities

Provide skilled nursing care in patients’ homes
Assess patients and identify physical, psychosocial, and environmental needs
Develop, implement, document, and evaluate the plan of care
Educate patients and families on care, diet, medications, and home safety
Coordinate care with the interdisciplinary team and referring agency
Supervise and provide clinical direction to home health aides and related staff
Manage pain, referrals, and third-party payer compliance
Perform on-call duties as assigned
