Home Health RN
Job Overview
- Contract role in Missoula, Montana
- Provides skilled nursing, care, and case management for patients in their residence or in an inpatient hospice setting
Required Qualifications
- Valid RN license in Montana
- Home Health experience within the last year
- 2 years of Home Health experience
- BLS
- Associate of Science in Nursing / Associate Degree in Nursing
- Driver's License
Preferred Qualifications
- BSN
Key Responsibilities
- Admit patients to services and coordinate care to support continuity and quality outcomes
- Assess, plan, implement, and evaluate nursing care using the nursing process
- Oversee care provided by Home Health/Hospice Aides and CNAs
- Maintain accurate patient records in accordance with agency standards and OASIS requirements
- Participate in team scheduling, on-call rotation, quality improvement, and in-service training
Schedule
- Schedule varies
- Typical in-home schedule is Monday through Friday, 8:00 AM to 5:00 PM, with on-call rotation
