RN Case Manager

Location

• Grand Junction, Colorado

Schedule

• Weekend coverage: 5 hours on one Saturday each month, with 5 hours taken off on one day during the week
• 36 hours per week

Required Qualifications

• Bachelor of Science in Nursing
• 5 years of Case Management experience
• Case Management experience within the last year
• Experience in a Hospital setting
• Discharge Planning
• Valid RN license in Colorado
• Current BLS

Preferred Qualifications

• Master’s degree in nursing
• Certification in case management, or willingness to obtain it within 2 years of employment

Key Responsibilities

• Develop, implement, and evaluate case management processes
• Assess, plan, coordinate, monitor, and evaluate patient health and human services needs
• Provide advocacy, communication, collaboration, resource management, and education to interdisciplinary team members
• Support appropriate, effective, and cost-efficient care for patients and populations
• Document case management plans and patient contacts clearly and concisely
• Collaborate with physicians, patients, families, staff, payers, and review organizations
• Educate physicians and staff on level-of-care and utilization issues
• Maintain knowledge of case management, discharge planning, and nursing issues through continuing education
