PT Behavioral Health

Overview

Travel/contract role in New York, New York
26-week assignment
20 hours per week
Day shift: Monday to Friday, 8:00 AM to 4:30 PM; hours are flexible

Required Qualifications

Valid Physical Therapist license in New York
Physical Therapist experience within the last year
1 year of experience as a Physical Therapist
ACLS - Advanced Cardiac Life Support issued by American Heart Association or American Red Cross
BLS - Basic Life Support issued by American Heart Association or American Red Cross
PALS - Pediatric Advanced Life Support issued by American Heart Association or American Red Cross

Required Screening and Documentation

State and national criminal background checks
Fingerprinting
Competency exam
2 references
Physical exam within 1 year of start
Negative TB test or chest X-ray within 12 months prior to start
Drug test within 1 year of start
Influenza vaccination or declination
Hepatitis B documentation, titer, or declination
Measles, mumps, rubella, and varicella vaccination documentation or titers
COVID-19 booster is preferred, not required

Training

TRUST training and annual standard trainings provided by the facility

Key Responsibilities

Perform initial evaluations and functional assessments
Develop individualized treatment plans
Provide physical therapy interventions
Monitor progress and adjust interventions as needed
Document evaluations, treatment, progress notes, and discharge summaries
Educate patients and direct care staff
Collaborate with physicians and other team members
Serve on Safe Patient Handling and Falls committees as needed
