Description

PT Behavioral Health

Overview

  • Travel/contract role in New York, New York
  • 26-week assignment
  • 20 hours per week
  • Day shift: Monday to Friday, 8:00 AM to 4:30 PM; hours are flexible

Required Qualifications

  • Valid Physical Therapist license in New York
  • Physical Therapist experience within the last year
  • 1 year of experience as a Physical Therapist
  • ACLS - Advanced Cardiac Life Support issued by American Heart Association or American Red Cross
  • BLS - Basic Life Support issued by American Heart Association or American Red Cross
  • PALS - Pediatric Advanced Life Support issued by American Heart Association or American Red Cross

Required Screening and Documentation

  • State and national criminal background checks
  • Fingerprinting
  • Competency exam
  • 2 references
  • Physical exam within 1 year of start
  • Negative TB test or chest X-ray within 12 months prior to start
  • Drug test within 1 year of start
  • Influenza vaccination or declination
  • Hepatitis B documentation, titer, or declination
  • Measles, mumps, rubella, and varicella vaccination documentation or titers
  • COVID-19 booster is preferred, not required

Training

  • TRUST training and annual standard trainings provided by the facility

Key Responsibilities

  • Perform initial evaluations and functional assessments
  • Develop individualized treatment plans
  • Provide physical therapy interventions
  • Monitor progress and adjust interventions as needed
  • Document evaluations, treatment, progress notes, and discharge summaries
  • Educate patients and direct care staff
  • Collaborate with physicians and other team members
  • Serve on Safe Patient Handling and Falls committees as needed
