Description

Hospice Care Operations

Job Overview

Contract length: 13 weeks
Employment type: Contractor
Schedule: Day shift, 40 hours per week
Shift times: Monday–Friday, 8:00 AM–4:30 PM; 10-hour shifts may also be available
Orientation: Up to 1 week, based on comfort level
Patient population: Adults only, end-of-life patients
Patient ratio: 1:1

Required Qualifications

Valid RN license in Wisconsin
Hospice Palliative Care RN with 2 years of experience
Hospice Palliative Care RN experience within the last year
BLS
Associate of Science in Nursing / Associate Degree in Nursing
Driver's License
Case Management experience

Preferred Experience

Hospice and EPIC Remote Client experience for first-time travelers
EPIC Remote Client experience preferred

Responsibilities

End-of-life care and planning
Wound assessments
Care coordination
Pain and symptom management
Triage

Schedule Notes

No weekends
No call
No holidays
Time off requests require manager approval

Work Setting

Floating may be required within the counties served
Scrubs: Any color or business casual
