Job Overview

- Shift: 4x10s, Sunday–Wednesday, 8:00 AM–6:30 PM
- Average daily census: 11
- Guaranteed hours: 40 hours/week
Required Qualifications
- Valid Physical Therapist license in Arizona
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- BLS issued by American Heart Association
Schedule / Work Expectations
- A weekend day is required
- May be asked to float to surrounding clinics
- First-time travelers are accepted
- Local candidates are accepted
Candidate Preferences
- Generalist with confidence treating medical diagnoses
- Ability to manage moderate to maximum assistance patients
- Comfortable with co-treatments
- Able to work with ventilated patients
- Able to adapt quickly and take additional evaluations as needed

- Unit manager approval is required for requested time off

- Background check required
- Education verification required
- Physical exam required within 1 year of start
- Drug screening required within 30 days of start
- TB screening required within 1 year of start
- COVID vaccination or declination required
- Flu vaccination or declination required
- Tdap documentation required
- Measles, mumps, rubella, varicella, and hepatitis B immunity or documentation required
- Photo for badge required
- Security question form and travel information verification must be completed before start
- Workforce member confidentiality agreement, substance abuse screening policy, professional attire and appearance policy, and unit-specific job description must be reviewed and signed before start
