Hospice RN

Required Qualifications
- Valid RN license in Utah
- Hospice (3 years of experience)
- Hospice experience within the last year
- BLS
- Associate of Science in Nursing / Associate Degree in Nursing
- Driver's License
Role Overview
- Provide skilled nursing care to terminally ill patients and support their families in accordance with the plan of care and hospice philosophy
- Perform comprehensive patient assessments and coordinate individualized care plans
- Manage pain and symptoms
- Educate patients and families about disease process, self-care, end-of-life issues, and hospice philosophy
- Collaborate with an interdisciplinary team
- Participate in on-call rotation
Preferred Experience
- Hospice, home health, or palliative care experience
Additional Details
- On-call coverage includes 1 to 3 shifts per week, including 3 weekends during the contract
- Mileage reimbursement is provided for home visits
