RN Case Manager

Monday-Friday, 8:00 AM-5:00 PM, with on-call and weekend rotation

Role Summary
RN Case Manager provides skilled nursing and case management for patients in their place of residence. The role may start as a Visiting RN and move to Case Manager, with the same clinical responsibilities in both designations.
Required Qualifications
- Valid RN license in Montana
- Hospice experience (2 years)
- Hospice experience within the last year
- BLS
- Associate of Science in Nursing / Associate Degree in Nursing
- Driver's License
- Home Health experience
Preferred Qualifications
- BSN
- 2 years of recent acute care, home care, or hospice experience
- Experience with venipuncture, catheterization, wound care, and/or parenteral therapy
- HCHB experience
Key Responsibilities
- Admit patients onto services
- Coordinate and integrate services to support continuity of care
- Assess, plan, implement, and evaluate nursing care
- Provide patient and family instruction
- Oversee care provided by Home Health Aides and CNAs
- Maintain accurate patient records and OASIS requirements
- Participate in team scheduling coordination and on-call coverage as needed
- Participate in quality improvement activities and in-service training
