Registered Nurse (RN) Case Manager - Hospice
Location: Fall River, MA (Territory-Based Position)
Patient Care Setting: Home Hospice
Contract Length: 13 Weeks
Schedule: Night Shift | 3 x 12-Hour Shifts | 36 Hours per Week
Shift: NOC (Night Shift)
Start Date: ASAP
On-Call Requirement: Yes
Meal Break: 30 Minutes
Cancellation Clause: 2 Weeks
Position Overview
We are seeking an experienced Registered Nurse Case Manager (RNCM) to provide compassionate, high-quality hospice care to patients within the Fall River territory.
The RN Case Manager will oversee patient care coordination, perform comprehensive assessments, manage caseloads, and deliver skilled hospice nursing services to support patients and families during end-of-life care.
Coverage Area: The territory is based out of the Fall River, MA office and currently includes:
- Fall River
- Dartmouth
- New Bedford
- Rochester
- Middleborough
- East Bridgewater
- Easton
- North Attleborough
- Mansfield
- Seekonk
- Swansea
- Somerset
- Taunton
- Surrounding communities and smaller towns within the service area
Key Responsibilities
- Manage a hospice patient caseload and serve as the primary clinical coordinator for assigned patients and families.
- Perform comprehensive hospice assessments, routine visits, symptom management visits, and clinical evaluations.
- Complete admissions and other case management functions as required.
- Collaborate with physicians, social workers, chaplains, hospice aides, and interdisciplinary team members to develop and implement individualized care plans.
- Provide pain and symptom management while supporting patients' comfort, dignity, and quality of life.
- Educate patients, caregivers, and families regarding disease progression, treatment plans, medications, and available hospice services.
- Monitor patient status, identify changes in condition, and communicate updates to the interdisciplinary team.
- Maintain accurate, timely, and compliant clinical documentation.
- Expected productivity: 24-30 patient visits per week based on a 40-hour equivalent workload.
- Active Registered Nurse (RN) license.
- Minimum recent Hospice Nursing Experience required.
- Prior Case Management Experience required.
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong assessment, communication, and care coordination skills.
- Ability to work independently while effectively managing a field-based caseload.
- HomeCare HomeBase (HCHB) experience strongly preferred.
- Experience with hospice admissions and case management.
- Strong end-of-life care and symptom management skills.
- Excellent organizational and time-management abilities.
- Ability to provide compassionate support to patients and families during challenging circumstances.
