Description
RN Care Manager
Overview
- Role: RN Care Manager
- Location: Chapel Hill, North Carolina
- Schedule: Days, 40 hours per week
- Contract length: 13 weeks
Summary
Provides ongoing support through assessment, planning, implementation, and evaluation of individual patient needs, with a focus on case management, utilization review, discharge planning, continuity of care, and cost effectiveness.
Required Qualifications
- Valid RN license in North Carolina
- Associate of Science in Nursing / Associate Degree in Nursing
- 2 years of Case Management experience
- Case Management experience within the last year
- Experience in an Acute Care setting
- Experience with discharge planning
- Care Manager experience
Preferred Qualifications
- BSN or MSN from an accredited school of Nursing
- Recent experience as a Case Manager, Care Manager, or Discharge Planner within an acute care hospital environment
Key Responsibilities
- Assess patient needs and document a plan of care in Epic
- Prioritize cases, identify new admissions, and monitor observation patients throughout the day
- Participate in daily care management touchpoints, CAPP meetings, and weekly Complex Care Meetings
- Coordinate with Social Work, Utilization Management, and the multidisciplinary team on discharge barriers, psychosocial concerns, and transition planning
- Support discharge readiness by identifying expected discharge dates, communicating medical milestones, and verifying understanding of the discharge plan
- Educate patients, families, and the care team on community resources and post-acute referrals
- Assist with medication resource management for non-resourced patients as needed
- Refer administrative tasks such as faxing and form processing to the Care Management Assistant
