Job Opportunity: RN Case Manager - Patient Care Teams
Location: Boynton Beach, Florida

Job Summary
As an RN Case Manager under physician's orders, you will identify and coordinate patient and family needs, providing preventative and supportive care to terminally ill patients and their families. Your role involves assessment, intervention, teaching, and maximizing patient comfort and quality of life.
Key Responsibilities
- Obtain patient history and perform age-specific physical, psychosocial, and spiritual assessments.
- Coordinate care and develop/update individualized Plans of Care with patients, families, interdisciplinary teams (IDT), and physicians.
- Identify patient and caregiver learning needs and apply appropriate teaching strategies.
- Promote safety by following policies, best practices, infection control, and ethical principles.
- Participate in interdisciplinary team meetings and collaborative care planning.
- Document care in compliance with organizational standards and policies.
- Provide support and closure contact following patient death.
- Supervise LPNs and Home Health Aides, including evaluating services and conducting supervisory visits.
- Maintain knowledge of relevant policies, state and federal regulations, and Joint Commission standards.
- Provide 24/7 patient care coverage as part of the team.
- Educate community members and IDT on hospice care philosophy and nursing care aspects.
- Administer, monitor, and document medications as prescribed.
- Attend mandatory trainings, in-services, and skills reviews.
- Maintain professional relationships with patients, families, caregivers, and community partners.
- Keep credentials and continuing education up to date, including BLS, licensure, and TB testing.
Required Qualifications
- Experience: Minimum 1 year of related nursing experience or equivalent education and experience.
- Preferred Experience:
- 1 year in acute or post-acute care settings.
- Experience caring for terminally ill patients or those with life-threatening illnesses.
- Education: ASN or ADN required.
- Licensure & Certifications:
- Valid Florida RN license.
- Valid Florida Driver’s License.
- Valid BLS certification (American Red Cross or American Heart Association).
- Other Requirements:
- Reliable transportation with valid automobile insurance and current MVR for field-based work.
- Ability to type 25 wpm is highly preferred.
