Job Description
Patient Care Responsibilities
- Lead nursing practice using the Professional Practice Model of Relationship Centered Care and Modified Primary Nursing to promote patient experience.
- Demonstrate proficiency and role model Purposeful Hourly Rounding (PHR) behaviors.
- Ensure integration of PHR into clinical activities to enhance Relationship Centered Care.
- Validate staff hourly rounds on all patients through direct observation and patient feedback.
- Assess staff competency in PHR and provide consistent, meaningful feedback.
- Conduct daily rounding of all patients (Monday-Friday).
- Communicate patient experience and staff engagement data to staff and interdisciplinary teams to develop and implement action plans.
- Collaborate with physician dyad and escalate concerns appropriately.
- Follow up on patient concerns promptly and provide service recovery as needed.
- Demonstrate caring, respect, compassion, empathy, and active listening through dialogue, body language, and actions.
Patient Experience
- Role model Relationship Centered Care through actions such as Admission Welcome, Bedside Shift Report, HELP, AM/PM Care, 5 Minute Sit Down, PHR, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration.
- Conduct staff huddles at shift start to identify high-risk patients requiring special attention.
- Communicate concerns identified during huddles and shifts to nursing leadership.
- Initiate service recovery for patients and families with concerns; escalate as needed.
- Interpret and communicate patient experience scores and benchmarks; keep staff informed of progress and opportunities.
- Consider patient values, preferences, cultural diversity, expressed needs, and knowledge in all care aspects.
- Empower staff to use evidence-based practices to improve understanding of patient perceptions.
- Demonstrate caring, respect, compassion, empathy, and active listening consistently.
Quality and Safety
- Design, support, and lead a safe, therapeutic, and efficient patient-centered care environment.
- Identify and investigate practice or operational issues by participating in Root Cause Analysis (RCA) and implement corrective actions.
- Foster a culture encouraging error and near-miss reporting.
- Lead interdisciplinary quality improvement teams using high reliability principles.
- Review unit progress, changes, and compliance with quality and safety metrics with clinical nurses and healthcare team members.
- Respond to escalations of safety hazards or best practice gaps by following institutional protocols.
- Monitor and address gaps in clinical nurse documentation related to institutional IT infrastructure.
- Lead staff participation in quality and safety initiatives and mandated compliance measures.
- Maintain accountability for all unit quality and safety practices and ensure staff competency evaluations are completed.
