Case Manager - Oklahoma City, OK
Job Summary
As a member of a multidisciplinary care management team, the nursing case manager is responsible for coordinating the care and services of selected patient populations.
Essential Responsibilities
- Assess patient goals including biophysical, psychosocial, environmental, discharge planning needs, and financial status.
- Monitor patient progress for changes in medical needs affecting treatment.
- Procure services and advocate for patients and families with limited resources.
- Work with third party payers to resolve issues affecting continuity of care.
- Coordinate and direct discharge planning with family, healthcare team, and community.
- Review admission data to determine appropriate level of care according to established criteria.
Contract Length
- 13 weeks
Required Qualifications
Experience
- Minimum 3 years clinical nursing experience in an acute care facility.
Education
- Graduate from an accredited school of nursing required.
Licensure & Certifications
- Current RN License issued by the Oklahoma State Board of Nursing or a current multistate Compact RN License (eNLC).
- Current Basic Life Support (BLS) certification issued by the American Heart Association required upon hire.
Skills & Knowledge
- Strong clinical assessment skills.
- Knowledge of nursing practices and procedures.
- Familiarity with community resources to support patient recovery.
- Understanding of third party payer reimbursement and regulatory issues.
- Ability to explain complex information clearly to patients and families.
- Strong interpersonal skills for gathering patient information and coordinating care.
Preferred Qualifications
- Bachelor of Science in Nursing (or higher) from a Board Approved Program preferred.
