Description

Job Opportunity: RN - Care Coordinator

Location: Minneapolis, Minnesota
Employment Type: Clinical Nursing
Contract Length: 30 weeks


Job Summary

The RN Care Coordinator provides patient- and family-centered care coordination across inpatient and outpatient settings. The role focuses on maximizing patient health outcomes and experience by identifying needs, supporting clinic visits, and ensuring smooth transitions of care. Responsibilities include coordinating care across the healthcare continuum, serving as a single point of contact, maintaining communication with primary/referring physicians, leading pre-visit planning, and facilitating patient/family education. The RN also manages medication reconciliation, documents visits, ensures authorizations and test results are in place, and supports proactive disease management using evidence-based practices.


Required Qualifications

Experience

  • Minimum 3 years of clinical nursing experience, preferably within a specific patient population group.
  • Proven ability to function at the highest level within scope of practice.
  • Experience with patient education and proactive disease management.
  • Excellent assessment and care coordination skills.
  • Experience using an Electronic Medical Record (EMR).
  • Strong computer and keyboarding skills.
  • Strong customer focus to accommodate patients.
  • Ability to follow protocol-specific care delivery expectations for the sub-specialty population.

Education

  • Completion of a BSN program within 4 years of hire.

Certifications & Licensure

  • Current Minnesota RN-BSN License OR ADN.
  • BLS certification (American Heart Association or American Red Cross).

Additional Information

  • Preferred: RN with 6+ years of surgical nursing experience (not required).
  • No explicit requirement for prior travel experience.

This position is ideal for experienced RNs interested in care coordination and patient advocacy within a multidisciplinary healthcare team.
