Job Opportunity: Hospice RN - Indiana (Fort Wayne, IN)
Job Summary
- Provide direct patient care to hospice patients and supportive care to their families.
- Manage an interdisciplinary team including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff.
- Case manage all assigned patient clinical records.
- Perform initial and ongoing patient assessments.
- Develop and implement Hospice Plan of Care in collaboration with physicians and interdisciplinary team.
- Provide comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
- Report changes in patient condition to attending and hospice physicians.
- Maintain accurate and updated medical records.
- Practice infection control and standard precautions.
- Monitor staff scheduling and participate in quality assessment and performance improvement programs.
- May participate in agency on-call schedule.
- Maintain knowledge of pain and symptom management in end-of-life care.
- Conduct supervisory visits with team members monthly.
- Ensure compliance with policies, procedures, and regulatory requirements.
- Maintain close contact with patient families to provide information and support.
- Act as primary liaison between physician, patient/family, and hospice team.
- Educate patients, families, and caregivers on care needs including medication administration, dressing changes, oxygen use, and hospice philosophy.

Required Qualifications
Experience
- Minimum 1 year of hospice experience within the last year.
- Experience in acute care, long term acute care, oncology, long term care, and home health settings.
Education
- Associate of Science in Nursing / Associate Degree in Nursing.
Certifications & Licensure
- Valid Registered Nurse (RN) license in Indiana.
- BLS certification issued by the American Heart Association.
- Valid driver’s license and current automobile insurance.
