Description

RN Case Manager - Patient Care Teams

Location: Ocala, Florida
Employment Type: Clinical Nursing - Hospice
Contract Length: 8 weeks


Job Summary

Under physician's orders, the RN Case Manager is responsible for identifying and coordinating patient and family needs, providing preventative and supportive care to terminally ill patients and their families. The role involves assessment, intervention, and teaching to maximize patient comfort and enhance quality of life.


Key Responsibilities

  • Obtain patient history and perform age-specific physical, psychosocial, and spiritual assessments.
  • Coordinate care and develop/update Plan of Care with patient, family, interdisciplinary team (IDT), and physician.
  • Identify patient and caregiver learning needs and provide tailored education.
  • Promote safety by applying best practice safety assessments and infection control principles.
  • Participate in care planning and coordination during IDT meetings.
  • Document care in compliance with organizational standards and policies.
  • Provide support following patient death in collaboration with the IDT.
  • Supervise LPNs and Home Health Aides, including evaluating services and conducting supervisory visits.
  • Maintain knowledge of relevant policies, state and federal regulations, and standards.
  • Provide 24/7 patient care coverage as part of the team.
  • Educate community members and IDT on hospice care philosophy and nursing aspects of patient care.
  • Administer, monitor, and document medications as prescribed.
  • Attend mandatory trainings, meetings, and skills reviews.
  • Maintain professional relationships with patients, families, caregivers, and community partners.
  • Maintain up-to-date credentials and continuing education, including BLS, licensure, and TB testing.

Required Qualifications

  • Minimum of 1 year of related nursing experience or equivalent education and experience.
  • ASN or ADN degree.
  • Valid Florida Registered Nurse license.
  • Valid Florida driver’s license.
  • Valid BLS certification from the American Red Cross or American Heart Association.
  • Reliable transportation with proof of valid automobile insurance and current motor vehicle record (MVR).

Preferred Qualifications

  • At least 1 year experience in acute or post-acute care settings.
  • Experience caring for terminal patients or those with life-threatening illnesses.
  • Ability to type 25 words per minute.

Additional Information

  • This position requires participation in 24/7 patient care coverage.
  • Must adhere to infection control and safety protocols.
  • Supervisory responsibilities include oversight of LPNs and Home Health Aides.
