Location: West Palm Beach, Florida

Contract Length: 10 weeks

Job Summary

As an RN Case Manager, you will provide supportive and preventative care to terminally ill patients and their families under physician orders. Your role includes assessing patient needs, coordinating care, and enhancing patient comfort and quality of life.

Key Responsibilities

• Obtain patient history and perform age-specific physical, psychosocial, and spiritual assessments.
• Coordinate care and develop/update individualized Plans of Care with patients, families, interdisciplinary teams, and physicians.
• Identify patient and caregiver learning needs and provide tailored education.
• Promote safety by applying best practice safety assessments and infection control principles.
• Participate in interdisciplinary team meetings and collaborative care planning.
• Document care in compliance with organizational standards and policies.
• Provide support and closure assistance following patient death.
• Supervise LPNs and Home Health Aides, including evaluating services and conducting supervisory visits.
• Administer, monitor, and document medications as prescribed.
• Maintain professional relationships with patients, families, caregivers, and community partners.
• Maintain up-to-date credentials and complete required continuing education, including CPR and TB testing.
• Participate in mandatory trainings, meetings, and skills reviews.

Required Qualifications

• Valid RN license in Florida
• Minimum of one (1) year of case management experience within the last year
• Hospice experience
• BLS certification issued by the American Red Cross or American Heart Association
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
