Registered Nurse - Hospice

Location: Morristown, New Jersey
Employment Type: Staffing - Supplier Sourced
Contract Length: 13 weeks (89 days ≈ 13 weeks)
Shift:

• 5 days per week, 8:00 AM to 4:00 PM
• On call or weekends required: 1 weekend per month
• Float to different counties as needed

Job Responsibilities

• Perform patient assessments to determine appropriateness for hospice care and develop individualized care plans for patients and families.
• Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement guidelines.
• Complete admission, discharge, and re-visit assessments and documentation within required timeframes.
• Use age-appropriate teaching methods and document patient education tools used.
• Manage case load by developing, implementing, and evaluating evidence-based plans of care; prioritize care needs and communicate progress with the interdisciplinary team (IDT).
• Attend and participate in IDT meetings every two weeks.
• Maintain a safe environment by following infection control measures and reporting infections.
• Demonstrate knowledge of pharmacology and safe medication management, including routine medication reconciliation and maintaining current medication orders.
• Participate in performance improvement programs, including reviewing health records and attending utilization review meetings.
• Provide orientation, mentorship, and cross-training to new employees and colleagues.
• Assist with agency growth by accepting additional assignments as requested.

Required Qualifications

• Minimum 2 years of experience as a Registered Nurse in a home hospice environment.
• Valid New Jersey driver’s license.
• Basic Life Support (BLS) certification.
• Experience using Electronic Medical Records (EMR).
• Excellent critical thinking and communication skills.

Preferred Qualifications

• 3 or more years of RN experience, preferably in home hospice.
