Hospice RN - Wisconsin
Location
- De Pere, Wisconsin
Job Summary
- Provide direct patient care to hospice patients and supportive care to their families.
- Manage an interdisciplinary team including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff.
- Case manage all assigned patient clinical records.
- Perform initial and ongoing patient assessments.
- Develop and implement Hospice Plan of Care in collaboration with physicians and interdisciplinary team.
- Provide physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
- Report changes in patient condition to attending and hospice physicians.
- Maintain accurate and updated medical records, including interdisciplinary notes and plans of care.
- Practice infection control and standard precautions.
- Monitor staff scheduling and participate in quality assessment and performance improvement programs.
- May participate in on-call schedule.
- Maintain knowledge of pain and symptom management in end-of-life care.
- Conduct routine supervisory visits with team members.
- Ensure compliance with policies, procedures, and regulatory requirements.
- Maintain close contact with patient families to provide support and continuity of care.
- Act as primary liaison between physician, patient/family, and hospice team.
- Educate patients, families, and caregivers on care needs including medication administration, dressing changes, oxygen use, and hospice philosophy.
Contract Length
- 13 weeks
Schedule
- Monday to Friday, 8:00 AM to 5:00 PM
- No on-call required
- Approximately 5-6 patients per day
- Visits include routine, death, PRN, and change in condition
- Travel radius up to 60 miles from branch
Required Qualifications
Education
- Associate of Science in Nursing / Associate Degree in Nursing
Experience
- Minimum 1 year Hospice Palliative Care RN experience within the last year
- Experience in Acute Care, Home Health, Long Term Care, and Oncology settings
Licensure & Certifications
- Valid Registered Nurse (RN) license in Wisconsin
- BLS certification issued by the American Heart Association
- Valid driver’s license
Skills & Knowledge
- Experience with Hospice Palliative Care nursing
- Use of electronic health record system (HCHB) required
- Ability to provide comprehensive hospice care including patient assessment, symptom management, and family support
- Strong communication and documentation skills
- Reliable transportation for work-related travel
