Description

Case Manager - Travel Position

Location: Morristown, New Jersey
Employment Type: Clinical Nursing - Travel

Job Description

Coordinate patient care throughout acute care hospitalization
Manage all aspects of discharge planning and transition process
Perform utilization management and quality screenings
In-patient case management required

Schedule

Shift: AM (8:00 AM - 4:00 PM)
Hours per day: 7
Hours per week: 37.5
Rotating weekends every 4th or 5th weekend
One summer and one winter holiday off

Contract Length

13 weeks

Required Qualifications

Valid Registered Nurse (RN) license in New Jersey
Bachelor of Science in Nursing (BSN)
Minimum 1 year of case management experience, including within the last year
Experience in an inpatient setting
Skilled Nursing Facility (SNF) experience
