RN Care Manager

Schedule

• 36 hours per week
• 12-hour shifts

Contract Length

• 13 weeks

Role Summary

Provide ongoing patient support through assessment, planning, implementation, and evaluation to improve care coordination, continuity of care, discharge readiness, and cost effectiveness.

Key Responsibilities

• Assess and document patient needs and care plans
• Prioritize new admissions and review daily worklists
• Participate in care management touchpoints, CAPP meetings, and weekly complex care meetings
• Coordinate with social work, utilization management, care management assistants, and the multidisciplinary care team
• Identify barriers to discharge and support transition planning
• Educate patients, families, and care teams on community resources and post-discharge needs
• Monitor observation patients and follow up on discharge-related action items
• Support medication resource management and authorization needs for post-discharge services

Required Qualifications

• Valid RN license in North Carolina
• Associate of Science in Nursing / Associate Degree in Nursing
• Case Management experience within the last year
• 1 year of Case Management experience
• Experience in an Acute Care setting
• Acute Hospital Case Management Experience
• Discharge Planning experience
• Care Manager experience

Preferred Experience

• Case management, utilization review, or discharge planning experience in an acute care hospital setting

Preferred Education

• BSN or MSN from an accredited school of nursing
