Case Manager

Job Overview

Location: Morristown, New Jersey
Schedule: 8:00 AM – 4:00 PM
Shift: Day
Hours per week: 37.5
Contract length: 13 weeks

Required Qualifications

Valid RN license in New Jersey
Bachelor of Science in Nursing
Case Management (2 years of experience)
Case Management experience within the last year
Experience in a Hospital setting
Experience in an Inpatient setting
2 years of acute care experience
Experience with subacute patients
Discharge Planning
Acute Hospital Case Management Experience

Preferred Qualifications

CCMC preferred

Role Summary

Coordinates patients through the acute care hospitalization
Manages discharge planning and transition processes
Performs utilization management and quality screenings

Schedule Notes

Rotating weekends: every 4th or 5th weekend
Holidays: one summer and one winter holiday
