RN Care Manager

Location

Required Qualifications

• Valid RN license in North Carolina
• Case Management (2 years of experience)
• Case Management experience within the last year
• Associate of Science in Nursing / Associate Degree in Nursing
• Experience in a(n) Acute Care setting
• Discharge Planning
• Utilization Review (InterQual)
• Acute Hospital Case Management Experience
• Care Manager experience
• Utilization Review experience

Preferred Education

• BSN or MSN from an accredited school of Nursing

Key Responsibilities

• Assess, plan, implement, and evaluate patient needs
• Support case management, utilization review, and discharge planning
• Review work lists to prioritize patients and identify new admissions
• Document assessments and care plans in Epic
• Participate in daily care management touchpoints, CAPP meetings, and weekly Complex Care Meetings
• Coordinate with social work, utilization management, care management assistants, and the multidisciplinary team
• Identify barriers to discharge, expected discharge dates, and readiness for discharge
• Support transition planning, post-discharge referrals, community resource education, and medication resource management
