Description

RN Care Manager

Location

  • Chapel Hill, North Carolina

Schedule

  • Days
  • 40 hours per week
  • 13 weeks

Role Summary

  • Provides ongoing patient assessment, care planning, implementation, and evaluation to support case management, utilization review, and discharge planning.
  • Focuses on care progression, discharge readiness, and coordination with the multidisciplinary team.

Required Qualifications

  • Valid RN license in North Carolina
  • Case Management (2 years of experience)
  • Case Management experience within the last year
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Experience in a(n) Acute Care or Hospital setting
  • Discharge Planning
  • Utilization Review (InterQual)
  • Acute Hospital Case Management Experience
  • Care Manager experience

Preferred Qualifications

  • BSN or MSN from an accredited school of Nursing

Key Responsibilities

  • Review work lists to prioritize patients and identify new admissions
  • Document assessment and plan of care in Epic
  • Participate in daily care management touchpoints
  • Attend CAPP meetings and Complex Care Meetings
  • Coordinate with social work, utilization management, and care management staff as needed
  • Identify barriers to discharge and support transition planning
  • Educate patients, families, and care teams on community resources and post-discharge needs
  • Monitor observation patients throughout the day for appropriate progression of care
  • Verify understanding and agreement with the discharge plan
