Description

Shift: Days | 8-hour shifts
Schedule: 40 hours/week | 8:00 AM – 4:30 PM
Weekends/Holidays: No weekends, no major holidays
Assignment Length: 4-month assignment

Required Certifications:

BSN

BLS

Experience Requirements:

Minimum of 3 years of RN experience functioning within a care management model of care delivery

Role Overview:
This position provides high-level nursing care management services utilizing geriatric nursing practice standards. Responsibilities include conducting comprehensive geriatric nursing assessments; developing, implementing, and reassessing individualized plans of care addressing both physical and psychosocial needs of older adults; triaging daily clinical issues; and delivering patient and family education via telephone. The role also involves coordinating care and services across multiple care settings to ensure optimal patient outcomes. The RN evaluates variances from established best practices and takes appropriate action to address them.
