Occupational Therapist (Acute Care) - Illinois

Location: Hoopeston, Illinois

Job Description

• Evaluate and treat patients of all ages.
• Serve as a role model in professional service delivery and as a clinical resource for staff and students.

Essential Functions

• Perform patient transfers with assistance as needed (bed, chair, floor).
• Use safe and appropriate body mechanics to reduce injury risk.
• Design and adjust plans of care integrating goals, treatment, outcomes, and discharge plans.
• Educate patients, families, caregivers, students, and healthcare providers using effective teaching methods.
• Evaluate clinical findings to determine therapy diagnoses and outcomes.
• Perform examinations using reliable and valid methods relevant to patient complaints and history.
• Screen patients to determine therapy needs and effectiveness.
• Utilize support personnel according to legal and ethical guidelines; monitor and provide feedback.
• Document evaluations, plans of care, goals, treatment modifications, and patient responses per policy.

Department-Specific Functions

• Implement intervention plans using functional activities considering patient/family emotional and functional needs.
• Modify approaches based on patient behavior and performance.
• Use standardized outcome measures consistently to reflect functional performance.
• Complete documentation in line with regulatory and organizational standards.
• Collaborate with patients, families, and significant others throughout rehabilitation; provide education and family training.
• Supervise COTA and rehabilitation technicians per regulatory requirements.
• Communicate patient status, activity/transfer recommendations, discharge disposition, and equipment needs with the medical team.

Required Education & Licensure

• Bachelor’s degree in Occupational Therapy
• Occupational Therapy (OT) license from Illinois
• Basic Life Support (BLS) certification
