Surgical Patient Navigator (Surgery/Oncology)

Job Summary

The Surgical Patient Navigator serves as an advocate and educator for patients and their families throughout the surgery and cancer care continuum.

Key Responsibilities

• Provide competent clinical care during office visits as a content expert in the specialty.
• Facilitate communication between patients and healthcare providers.
• Educate patients and families, providing necessary resources.
• Greet patients courteously and efficiently process their information.
• Complete physician’s orders accurately and thoroughly.
• Guide patients through the healthcare system and advocate on their behalf.
• Support patients and families in coping with psychological aspects of cancer.
• Coordinate appointments for diagnostic and therapeutic services, accommodating patient needs.
• Manage patient telephone calls, including triaging urgent calls and scheduling.
• Communicate professionally with physicians and relay messages to clinicians.
• Ensure all necessary materials (films, pathology, lab reports) are available for appointments.
• Advise patients on records and forms needed for visits.
• Prepare new patient records according to practice standards.
• Elicit clinical information to schedule referrals appropriately.
• Encourage referrals and schedule new appointments based on clinical presentation and protocols.
• Investigate scheduling options if next available appointment exceeds reasonable timeframe.
• Identify and address barriers to timely and appropriate care.
• Connect patients with resources, social support, and financial services.
• Collaborate with clinical research teams to identify potential clinical trial candidates.
• Network with hospital and community services.
• Provide community education to raise public awareness.
• Participate in practice hours and locations as needed.

Required Skills & Experience

• Minimum 5 years nursing experience in Surgery or Oncology.
• Proficient computer skills.
• Excellent written and verbal communication skills.
• Strong organizational and critical thinking skills.

Education & Licensure

• Registered Nurse (RN) with Associates, Diploma, or BSN degree.
• Valid New Jersey Nursing License.
