Job Summary
Under the supervision of the Assistant Director of Cardiology, the Exercise Physiologist will:
- Assess, plan, implement, evaluate, and supervise individualized care and group exercise sessions in the Cardiac Rehabilitation Department.
- Provide patient education on cardiac diseases (heart failure, coronary artery disease, post-MI, post-surgery), diabetes, and other chronic diseases.
- Individualize patient care based on age and developmental needs.
- Collaborate with physicians, patients/families, and healthcare team members to deliver and coordinate care plans.
- Demonstrate knowledge and skills appropriate to the age of patients served.
Primary Duties
- Complete intake assessments with Cardiac Rehab Staff RN and document patient records.
- Develop and coordinate individualized plans of care with nursing and medical staff.
- Participate in discharge planning and patient/family teaching.
- Provide diabetes self-management education (DSME) targeting patients at risk or diagnosed with Type I and Type II diabetes, following evidence-based standards from ADA and AADE.
- Conduct individual assessments and develop strategies for diabetes self-management, including psychosocial, nutritional, and exercise interventions.
- Reassess diabetes self-management progress every 30 days or as needed.
- Document all diabetes education in the patient’s Individual Treatment Plan.
- Evaluate and update patient responses to care plans.
- Participate in inservices, committees, and performance improvement activities.
- Facilitate physiological, psychosocial, and spiritual care of patients.
- Use positive communication skills for conflict resolution and teamwork.
- Monitor use of time, equipment, and supplies to contribute to cost-effective care.
- Comply with safety policies to maintain a safe environment for staff, patients, and visitors.
- Demonstrate flexibility with changes in hospital or unit activities.
- Actively participate in achieving departmental goals.
- Responsible for exercise prescription assessment and interpretation of heart rate and blood pressure during exercise therapy.
- Recognize and report symptoms related to cardiac, respiratory, and musculoskeletal systems to RN.
- Participate as a member of the Code Blue Team under direction of the team leader or Cardiac Rehab Staff RN.
- Communicate accurately and timely with staff, patients, and physicians regarding patient progress and treatment.
- Complete all required documentation including progress notes.
- Flex hours as needed during staff vacations or for education and meetings.
- Serve as a role model for patients.
