Registered Nurse - Extended Care
Location: Miles City, Montana
Employment Type: Staffing - Supplier Sourced
Contract Length: 13 weeks (approximately 3 months)
Job Summary
- Provide holistic nursing care focusing on the individual’s role within their family, culture, and society.
- Utilize the nursing process: assessment, diagnosis, planning, implementation, and evaluation.
- Advocate for residents by ensuring they and/or their representatives have adequate information to make health decisions.
- Coordinate total care to help residents achieve or maintain their highest practicable level of function.
- Maintain quality of resident care in compliance with legal and professional standards.
- Serve as a role model to other healthcare team members.
Essential Duties and Responsibilities
Assessment & Planning
- Complete age-specific admission history, physical assessments, and nursing care plans for assigned residents.
- Analyze clinical data (labs, x-rays, diagnostics) to develop individualized care plans.
- Identify and document residents at risk for nutritional or functional decline.
- Recognize and report signs of neglect or abuse (child, domestic, elder).
- Set priorities and review daily care plans in collaboration with residents, families, and healthcare team.
- Delegate tasks based on resident needs and staff competencies.
- Communicate with physicians to coordinate medical and nursing care plans.
Implementation
- Demonstrate competence in technical nursing skills.
- Manage urgent and emergent resident care situations proactively.
- Administer medications and IVs according to scope of practice and policy.
Evaluation
- Document nursing interventions, resident responses, and outcomes.
- Revise care plans based on ongoing evaluation.
- Assess responses to interventions against age-appropriate norms.
Patient Education & Continuum of Care
- Identify and document learning needs of residents and families; provide education accordingly.
- Identify and document discharge planning needs.
- Coordinate or delegate discharge activities including documentation and instructions.
- Maintain knowledge of CMS regulations, Medicare skilled care, and MDS criteria to ensure compliant documentation.
- Incorporate resident preferences into care delivery.
- Supervise, coach, counsel, and evaluate assigned CNA staff.
Required Qualifications
- Registered Nurse (RN) license appropriate for Montana.
- Ability to read and write at a level sufficient for documentation requirements.
- Physical ability to perform job duties including standing, walking, sitting, pushing, pulling, lifting (frequent lifting/carrying up to 50 lbs, occasional lifting over 100 lbs), stooping, kneeling, crouching, reaching, filing, typing, and talking.
- Hearing and color vision adequate for job functions.
