Virtual Care Coordinator

Location: New York, NY
Employment Type: Contract
Contract Length: 13 weeks
Shift: 7 hours/day, 37.5 hours/week

Job Summary

The Virtual Care Coordinator supports virtual patient care activities and promotes patient-focused care through remote monitoring and telehealth services. Responsibilities include patient safety monitoring, telehealth platform support, collaboration with clinical teams and technology partners, quality assurance, and training. The role requires clear communication, teamwork, and accountability for clinical quality and safety outcomes.

Key Responsibilities

• Perform quality assurance and peer reviews of telehealth and virtual patient observation programs.
• Collaborate with leadership to evaluate program metrics, develop policies, and implement workflow improvements.
• Triage and assign cases for virtual monitoring and observation.
• Maintain operational databases tracking telehealth program performance and quality metrics.
• Identify trends and participate in corrective action plans.
• Provide training on telehealth workflows, platform use, and documentation to new staff.
• Serve as liaison between clinical staff, physicians, technology partners, and vendors regarding telehealth platform issues.
• Facilitate virtual patient care by analyzing utilization reports and coordinating patient transitions.
• Provide real-time support to clinical staff for telehealth and remote monitoring equipment troubleshooting.
• Continuously observe assigned patients for clinical status changes, safety risks, and fall prevention.
• Monitor early warning signs such as vital signs and device days (e.g., central line, Foley catheter).
• Document patient behavior, interventions, and escalate concerns to clinical teams promptly.
• Monitor telehealth consult orders and ensure sessions start and complete as scheduled.
• Maintain patient privacy and adhere to HIPAA and information security policies.
• Escalate patient safety concerns and equipment issues as needed.
• Maintain accurate records of interventions and safety events.
• Participate in staff meetings, team briefs, and maintain punctuality and attendance records.
• Maintain a safe, clean, and functional work environment.

Required Qualifications

• Minimum 2 years of Care Coordinator experience, including experience within the last year.
• Experience as a Monitor Technician.

Education

• Associate or Bachelor’s degree in a health-related field preferred.

Additional Information

• Must provide “White Glove Service” by offering real-time support to clinical staff.
• Responsible for ensuring telehealth sessions run smoothly without technical issues.
• Must maintain confidentiality and comply with all privacy and security guidelines.
• Ability to prioritize, problem-solve, and exercise good judgment in complex situations.
