Virtual Care Coordinator

Location: New York, NY
Contract Length: 13 weeks
Shift: 7 hours/day, 37.5 hours/week

Job Summary

The Virtual Care Coordinator supports virtual patient care activities and promotes patient-focused care through remote monitoring, telehealth services, and hospital environmental status monitoring. This role involves collaboration with clinical teams, leadership, technology partners, and vendors to implement telehealth programs, ensure quality and safety, and provide training and support.

Key Responsibilities

• Perform quality assurance, peer reviews, and evaluate telehealth and virtual patient observation programs.
• Collaborate with leadership to review program quality metrics and recommend workflow improvements.
• Lead case reviews and implement solutions with hospital partners.
• Triage and assign cases for virtual monitoring/observation.
• Develop and implement telehealth policies and workflows.
• Maintain operational databases tracking telehealth program performance and quality metrics.
• Identify trends and participate in corrective action plans.
• Provide training on telehealth workflows, platform use, and documentation to new staff.
• Serve as liaison between clinical staff, physicians, technology partners, and vendors regarding telehealth platform issues.
• Facilitate camera utilization and coordinate patient eligibility for virtual care.
• Provide real-time support to troubleshoot telehealth and remote monitoring equipment and platform issues.
• Continuously observe assigned patients for clinical status changes, safety, and fall prevention as directed by registered nurses.
• Monitor early warning signs such as vital signs and device days (e.g., central line, Foley catheter).
• Communicate effectively with patients, visitors, and clinical teams; report any clinical concerns immediately.
• Document patient behavior, interventions, and escalate high-risk situations to clinical teams.
• Monitor telehealth consult orders and ensure sessions start and complete as scheduled.
• Maintain patient privacy in compliance with HIPAA and organizational policies.
• Escalate patient safety concerns and equipment issues promptly.
• Maintain accurate records of interventions and safety events.
• Participate in staff meetings, team briefs, and stay updated on new initiatives.
• Maintain a safe, clean, and functional work environment.

Required Qualifications

• Minimum 2 years of Care Coordinator experience, including experience within the last year.
• Experience in direct patient care.
• Experience as a Monitor Technician.

Education

• Associate or Bachelor’s degree in a health-related field preferred.

Additional Notes

• Must complete Virtual Care Coordinator orientation.
• Prior travel experience is not specified or required.
