Job Title: Virtual Care Coordinator

Job Summary: As an integral part of a team, the Virtual Care Coordinator is responsible for supporting virtual patient care activities and promoting patient-focused care within the Mount Sinai Health System. This role involves patient monitoring for safety and clinical needs, supporting hospital-based telemedicine services, and monitoring hospital environmental status. The Virtual Care Coordinator collaborates on implementing telehealth and remote patient monitoring programs, coordinates training, and participates in developing telehealth best practices. The role requires clear communication and collaboration with clinical teams, patients, Digital Technology Partners, engineering, telehealth platform vendors, and other stakeholders. The coordinator shares accountability for clinical quality and safety outcomes.
Required Qualifications:
- Minimum three years of direct patient care experience.
- Experience creating policies impacting telehealth.
- Experience with quality, outcomes, and data collection.
- Proven ability to analyze legislative, regulatory, and organizational policies affecting telehealth.
- Experience identifying data elements for quality improvement/assurance.
- Developed efficient clinical telehealth workflows.
- Experience drafting organizational protocols for telehealth.
- Experience developing strategies for effective telehealth program development.
- Proven ability to measure and communicate program successes and lessons learned.
- Experience identifying changes in patient clinical status and behavior requiring escalation.
- Experience identifying patient behaviors that pose safety risks.
- Collaboration experience with interdisciplinary teams.
- Experience with telehealth or remote monitoring technology.
- Experience with electronic medical records.
- Knowledge of telehealth technologies and their appropriate uses.
- Ability to identify benefits and barriers to implementing telehealth.
- Experience communicating telehealth concepts to organizational members and healthcare consumers.
- Proven ability to identify elements of successful telehealth encounters and quality telepresence.
- Ability to consider language, culture, and disability in using telehealth technologies.
Preferred Qualifications:
- Associate or Bachelor’s degree in a health-related field.
Additional Information:
- Successful completion of Virtual Care Coordinator orientation.
- Ability to set priorities, problem-solve, and use proper judgment in difficult situations.
- Strong oral and written communication skills in English.
- Maintains attendance and punctuality records per department policies.
- Adheres to MSHS information security policies and HIPAA guidelines.
