Description

Job Title: Care Manager - ED (000087)

Location: State of New York

Job Summary: The Care Manager - ED is responsible for providing comprehensive care coordination, utilization review, and discharge planning for patients in the emergency department. This role requires a professional with strong clinical expertise and computer literacy to ensure effective patient care and compliance with healthcare standards.

Required Qualifications:

Five (5) years minimum of clinical experience as a Registered Nurse (RN).
Experience with care coordination, utilization review, and discharge planning.
Computer literacy.

Preferred Qualifications:

Leadership experience.

Required Education:

Associates Degree (ASN, ADN) from an accredited nursing program.

Preferred Education:

Bachelor's or Master's degree in Nursing.

Required Certifications & Licensure:

RN licensed as a Registered Nurse in the State of New York.
PRI and Screen certification within three months of employment.
Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED).
BLS certification.

Preferred Certifications & Licensure:

Case Management Certification.
