Travel Registered Nurse – Case Management| $2960 weekly (Stipend package option available to help maximize your take-home pay) |40 hours/week
We are seeking an experienced RN Case Manager to float between Walnut Creek and Concord campuses (approximately 8 miles apart).
Job Details:
Location: Walnut Creek, CA
Contract Duration: 13 weeks
Shift: 8:00 AM – 4:30 PM
Will work every other weekend (EOW) or
Set rotation: Every Tuesday–Saturday36 hours/week
Responsibilities
Provide case management services in an acute care hospital setting
Coordinate patient care, utilization review, and discharge planning
Collaborate with physicians, nursing, social work, and interdisciplinary teams
Assess patient needs and facilitate transitions of care
Ensure compliance with hospital policies, payer requirements, and regulatory standards
Document case management activities accurately in the EMR
Float between campuses as assigned
Requirements
Active California RN license required
Minimum 2+ years of recent acute care hospital case management experience
Ability to float between two hospital campuses
