As part of the person centered care model, this role will develop/coordinate agency wide programs for prevention and management of skin breakdown. Provides expert consultation and/or hands-on care for individual patients with acute or chronic wounds. Establishes/coordinates multi-disciplinary, comprehensive management plans. Coordinates care for populations of patients, identifying trends, and developing standardized prevention and treatment protocols and refining protocols based on outcomes. Develops and implements educational programs for patients, staff and other health care providers. Coordinates quality improvement initiatives to optimize skin and wound outcomes. Advocates for the patient with skin and wound care needs.

Manages regional wound care program metrics ensuring consistent delivery of quality care and communications; collaborates and explores opportunities for cost effectiveness for wound treatments in relationship to patient needs 

• Reviews Quality reporting data for deficiencies, areas for improvement, barriers to safety 

• Seeks consultation with other regional WOCN staff to identify and plan interventions designed to solve complex problems involving client care. 

• Develops an interdisciplinary approach to client care between Wound program, HHCAH clinical staff and community partners. The Wound Care Specialist will act as consultant to help coordinate all client care services with community partners and agencies as required for the patient to achieve outcomes. Informs the physician/wound clinics of changes in client needs, wounds that require change to the plan of care for wound care treatments and disease management 

• Follows trends in specialty area of wounds for home care industry; makes recommendations of treatment options for incorporation into the program.

• Participates in reviewing regional wound program performance metrics to ensure program is achieving desired clinical and operational performance measures with Regional Director and Manger of Specialty Services.

• Participates in training, coaching of staff and onboarding to the Wound program

• Participates and facilitates professional development needs and competency for wound program.

In addition, the position involves other duties such as: 

Will actively participate in the performance improvement process known as H3W.

Participates in clinical education area to provide programs aimed to improve clinical competencies of direct service staff. 

Researches new information related to wound specialty and communicates to team. 

Performs other duties as assigned.


Qualifications


Education: Bachelor’s Degree in nursing required. WOCN certification required. MSN preferred.

Experience: Minimum of two years nursing experience in clinical specialty area required. Minimum two years in homecare preferred.

Licensure, Certification, Registration: RN required. Obtain Wound/Ostomy certification within 6 months of hire required.

Language Skills: Strong written and verbal communication skills.

Knowledge, Skills and Ability Requirements: 

• Ability to effectively communicate at all levels within the organization and share knowledge, ideas and information. 

• Demonstrated success in management, planning and leadership ability for meeting patient outcomes. 

• Able to understand problem situations, solve problems and independently assess a wide variety of tasks in order to effectively take action to identify solutions to benefit the client/agency 

• Knowledge of relevant industry standards for wound care treatments 

• Ability to balance financial, quality, people and customer expectation appropriate to business situation. 

• Intermediate to advanced MS Outlook, Word, Excel and PowerPoint, and Microsoft Project.
