Registered Nurse Care Manager who pro-actively manages the care for patients along a continuum by facilitating collaborative care between patient, family/support system, physicians, and other ancillary departments/members of the primary care team.

Type of Opportunity: Full time FTE: 1.00 Job Exempt: No Work Shift: Days (United States of America)

Responsibilities:

• Assessment of an identified patient population based on symptoms and medical history.

• Development of a plan of care based on preventative care needs, using standard based practice guidelines.

• Collaborates with patients to update problem list, current medication list and coordinates visit plan.

• Serves as patient advocate. Serves as communication link between Inpatient Care Manager, HHC Care Manager and PHP Case Manager.

• Documentation within scope of practice.

• Other duties as required

Qualifications:

• Associate’s degree in nursing

• State of New Mexico or Compact State Nursing License

• BLS certification REQUIRED at at time or hire

• 1-3 years ambulatory experience
