Job Summary and Qualifications 

This position supports the mission of Swedish Medical Center of optimizing the quality of life of all those served by expanding beyond the traditional focus on medical care to meet the needs of the whole person. The Clinical Coord Case Manager is responsible for delivery of care coordination and delivery of quality patient care with cost effective utilization of resources. This position supports collaboration and communication among the patient/family, physician and the entire health care team regarding the patient’s plan of care and continued needs post-discharge. The Clinical Coord Case Manager will communicate daily with the discharge planners, social work staff, and UR RN’s to determine the plan of care and communicate this to the physicians. Communication with other members of the interdisciplinary team will be ongoing to determine the post-acute care needs of the patient. The communication with post-acute vendors; LTAC, sub-Acute Rehab, SNF, HHC etc. will be critical to ensure appropriate and timely referrals are initiated. The Clinical Coord Case Manager will be the primary contact for the discharge planners and the social workers to discuss issues that arise with patients, families, and physicians. Avoidable days will be recorded as a team effort with UR nursing, DCP and SW staff. The Clinical Coord Case Manager’s will be required to report on length of stay issues and barriers to discharge with administration on a weekly or as needed basis.

II. Position Requirements:

A. Licensure/Certification/Registration:

Current licensure in the State of Colorado as a Registered Nurse, or current active multistate nursing licensure. BLS certification through AHA or ARC.

B. Education:

Graduate of an accredited school of nursing; BSN preferred.

C. Experience:

3-5 years recent clinical experience in an acute care setting. 5-7 years recent clinical experience in an acute care setting preferred. Minimum of two years of supervisory experience required.

D. Knowledge/Skills/Abilities:

Requires excellent human relations and oral/written communication skills. Must have the ability to asses’ information, apply critical thinking processes to determine course of action, define expected outcomes, and evaluate effectiveness of plan through continuous assessment of the patient's progress.

Must have the ability to compile, evaluate and report statistics to members of the team, and utilize the information to facilitate process improvement activities. Strong communication skills are necessary for meeting facilitation, educational presentations, team collaboration and patient interviews. Must also have strong analytical and organizational skills.
