Work Schedule:
90% FTE, 12 hr. shifts, day/evening shift with required on-call for off hours. Weekend and holiday rotation required. Hours are based off the operational needs of the department. This position is primarily onsite working at 301 S. Westfield Rd. and Northport Dr. Clinic, both located in Madison, WI. Travel may be required to University Hospital and East Madison Hospital to enroll patients into the program. Travel to patients’ homes may also be required (within a 30-mile radius of University Hospital).
Pay:
• Additional components of compensation may include:
o Shift Differentials: $4/hour evening and $5/hour night as applicable for hours worked.
o Weekend Differential: $4.25/hour as applicable for hours worked.
o Overtime
o On-call pay
• Relocation assistance may be available for qualified applicants.

We are seeking a Registered Nurse (RN) - Care Team Lead/Charge Nurse to:
• Provide front-line nursing leadership with oversight of Home-Based Hospital Care (HBHC) RNs and Mobile Integrated Health Professionals (Paramedics), motivating and mentoring teams delivering acute-level care to patients in their homes through innovative, hospital-at-home models.
• Lead and coordinate daily operations by assessing staffing and resource needs, collaborating with clinical and nonclinical partners, and coordinating admissions, transfers, and discharges to ensure safe, efficient, and timely care delivery.
• Serve as a clinical and operational expert, applying advanced assessment, critical thinking, and evidence-based practice to support quality outcomes, patient safety, and continuous improvement across the HBHC program.
• Partner with the interdisciplinary team—including providers, nurse managers, social work, and ancillary services—to establish care goals, support discharge and post-acute planning, and ensure nurses practice at the top of their license.
• Utilize transformational leadership and emotional intelligence to foster accountability, professionalism, and staff development while serving as a role model, clinical resource, and coach for nursing staff and learners.
• Support innovative care delivery through digital health technology, interdisciplinary conferencing, and high-quality documentation, advancing the mission and strategic goals of UW Health’s Home-Based Hospital Care program.

Qualifications
• Bachelor's Degree in Nursing (BSN) Preferred
Work Experience
• 2 years experience in an inpatient unit Required
• 2 years of combined work experience and leadership experience may be considered in lieu of two (2) years’ experience in an inpatient unit Required
• 3 years experience in an acute care inpatient unit Preferred
• Relevant RN experience with department-specific patient population Preferred
Licenses & Certifications
• Licensed as a Registered Nurse (RN) in the State of WI or holds a license issued by a jurisdiction that has adopted the nurse licensure compact Required
• Current CPR/BLS certification Required
• Must possess a current, valid driver’s license issued by the individual’s state of residence.
• Must be at least 21 years old and have a minimum of three (3) years of driving experience to operate a Personal Vehicle, Patient/Visitor Vehicle, or standard UW Health Fleet Vehicle. Required
• National certification in relevant specialty Preferred
