Work Schedule:
Day/evening, you will work five10-hour shifts per pay period including every other weekend. Holiday rotation required. Hours are based off the operational needs of the department. This position is primarily onsite at University Hospital and East Madison Hospital with travel between sites as operationally needed.
Pay:
Additional components of compensation include impressive shift differentials - $4/hour evening, $5/hour night and $4.25/hour weekend differential as applicable for hours worked.

Be at the forefront of an innovative hospital-at-home model by identifying and enrolling appropriate patients at University Hospital and East Madison Hospital into the Home-Based Hospital Care (HBHC) program.
We are seeking a Registered Nurse to:
- Conduct bedside intake, eligibility screening, and informed consent discussions to confidently transition patients from traditional inpatient care to acute care at home.
- Partner daily with the admitting physician to proactively review patient lists, identify top candidates, and coordinate seamless, timely discharges to home-based hospital services.
- Prepare patients for departure by arranging transportation, coordinating equipment and appropriate care supplies, and ensuring a smooth handoff to the HBHC interdisciplinary team.
- Serve as a key connector between hospital and home, collaborating closely with providers, nursing teams, and support services to launch each patient’s HBHC episode successfully.
- This primarily onsite, hospital-based role offers autonomy and collaboration, with shared workspace at East Madison Hospital and dedicated space at University Hospital; travel between sites may be required as operationally needed.

- Bachelor's Degree in Nursing (BSN) Preferred
- 2 years relevant acute care or home health nursing experience Required
- Experience in care coordination, triage, or nursing leadership Preferred
- Experience in program development Preferred
- Experience in teaching self-care skills to patients and families Preferred
- Recent, relevant home health experience Preferred
- Experience using EPIC and virtual health platforms Preferred
- Experience with IV therapy and knowledge of venous-access devices and ambulatory pumps Preferred
- Licensed as a Registered Nurse (RN) in the State of WI or holds a license issued by a jurisdiction that has adopted the nurse licensure compact Required
- Current CPR/BLS Certification Required
- Relevant certification Preferred
