100%, day shift. You will work at University Hospital and Eastpark Medical Center in Madison, WI.

As an Advanced Practice Provider, you'll play an integral role in our inpatient and ambulatory care teams by providing high-quality, cost-effective care to patients.
We are seeking a Nurse Practitioner or Physician Assistant to:
- Work collaboratively within a multidisciplinary health team to provide comprehensive care to cancer patients in active treatment or active surveillance, in a primarily outpatient setting.
- Provide follow up and urgent appointments at University Hospital and Eastpark Medical Center.
- Perform routine clinical activities independently as outlined in practice agreements/scope of practice including, but not limited to, the following: clearance assessment for cancer treatment; symptom assessment, evaluation, and management; assessing cancer patients on clinical trials; as well as obtain chemotherapy signing privileges.

Education:
Minimum –
If Nurse Practitioner:
Master’s or Doctorate’s degree from accredited Nursing Program.
If Physician Assistant:
Bachelor’s or Master’s degree from accredited PA Program.
Licenses and Certifications:
Minimum -
If Nurse Practitioner: Active RN and APNP license in State of Wisconsin AND active board certification as a NP in Adult; Adult/Gerontology Primary Care, Adult/Gerontology Acute Care.
If Physician Assistant: Active PA license in State of Wisconsin AND active PA board certification.
All Nurse Practitioner and Physician Assistant applicants must have: Active DEA license or ability to apply for such license prior to or upon hire AND CPR certification and advanced life support certifications if relevant to practice area (ACLS, PALS, NRP).
