Will commute to patients on a daily basis throughout designated region. Regional Home Health Physical Therapist for LHC Group. Nurse will have a case load of up to 6 patients a day throughout their designated region. Patient care requirements can vary. Based on home health experience and skill sets, the nurse will be matched to provide the relevant care. The more versatile the range of home health skills the better. CURRENTLY 3 Regions. Please specify which region/county Nurse wants. Polk/Central Florida Tampa/Pinellas/Pasco Bradenton/Fort Myer- SW FloridaCharting System:HCHB and OasisClient Interview Required:YesClient Interview Details:YesPay / Compensation:  Local - $63/hr +   | Travel (over 50mi) - $2600+Avg Max Ratio/Caseload:Max 6 cases per day

The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician’s orders when applicable.

Minimum Requirements:

• Current licensure as a Physical Therapist in the state of practice
• Graduate of an accredited school of Physical Therapy
• One (1) year of prior professional Physical Therapy experience preferred
• Current CPR if applicable
• TB Questionnaire, PPD or chest x-ray if applicable 
• Current Health certificate (per contract or state regulation) 
• Must meet all federal, state and local requirements 
• Must be at least 18 years of age
